Plain-English Summary of What We Found

  • Competing medications affecting thinking and memory: Your mom takes Donepezil to help her memory. However, the over-the-counter sleep medicine she’s been taking (Benadryl / Tylenol PM) does the exact opposite—it blocks the memory chemical in her brain. These two medicines are fighting each other, which is a major reason for her increased confusion and dry mouth.
  • The Fall Danger: Combining Lorazepam (Ativan) with OTC sleep aids creates a heavy sedating effect that lingers into the next day. This explains her daytime sleepiness and unsteady walking.

What You Should Do Immediately (No Prescription Needed)

  1. STOP Giving OTC Benadryl & Tylenol PM: Remove these from her nightstand today.
  2. For Nighttime Joint Pain: Use plain Tylenol (Acetaminophen) Extra Strength (500 mg per pill). Do not give more than 2 pills in 24 hours without doctor approval.
  3. Change the Water Pill Time: Give her Hydrochlorothiazide (HCTZ) in the morning with breakfast, not at night. This will reduce midnight bathroom trips.
  4. Sleep Hygiene Improvements: Keep her room dark and cool, limit liquid intake after 7:00 PM, and encourage 15 minutes of morning sunlight.

⚠️ CRITICAL SAFETY WARNING

DO NOT stop Lorazepam (Ativan) suddenly on your own. Stopping anxiety medications abruptly can cause dangerous withdrawal symptoms, severe agitation, or seizures. This must be stepped down slowly under her doctor’s supervision.

What to Bring to the Doctor’s Appointment

  • Bring all her medication bottles in a clear zip bag (including OTCs).
  • Hand the physician the "Clinician Discussion Guide" (Version A) at the start of the visit.
  • Track her daily confusion and alertness levels on a notepad for 7 days before the appointment.

"Doctor, we had a clinical pharmacist review Mom's medication list to look for factors affecting her memory and balance. We learned her OTC sleep aid is directly blocking her dementia medication. We have stopped the OTC sleep aid—could we review a safe plan today to gradually taper her Lorazepam and adjust her diuretic timing?"
